Gugan Gulwan Youth Aboriginal Corporation acknowledges the Traditional Custodians of the lands on which we walk, work, live and learn. Case Management
Gugan Gulwan provides comprehensive case management. Two workers, often from different teams within Gugan Gulwan, assist all clients as they do not present with just one issue. Gugan Gulwan Case Management uses the strength of family, community, and culture as part of an individualised case plan. All case management plans are developed with the clients, their families, and any other important people nominated by the client to help them achieve their case management goals. Case management plans are frequently assessed and redesigned as the client reaches or changes goals. Where there is a referral provided to a client, Gugan Gulwan practices warm referrals and continues wrap-around case management until the client has engaged with the other service. This assists workers to ensure clients do not slip through the gaps.
